Step 3: Reference Forms
Your application can only be finalized when your three reference forms are completed and on file with your application. Please feel free to submit your application prior to having your references completed. There are two ways to have your reference forms completed.
- Print 3 reference forms and give them to your references, who will send them to Black Diamond Kids Center when they have completed them.
